New Delhi, Feb. 7 -- The death of 25-year-old Kamal Dhyani has exposed a fatal disconnect between official safety mandates and their implementation on the ground, revealing how repeated warnings issued after a similar recent tragedy in Noida were systematically ignored. On January 24, the Delhi Jal Board issued an order commanding engineers and contractors to "ensure adequate safety arrangements at all construction and excavation sites". Similar guidelines were issued by PWD and I&FC Department. Despite this, the Janakpuri site where Dhyani died appeared to be catastrophically unsecured.P5
