According to the abstract* released by the U.S. Patent & Trademark Office: "A replacement duct sleeve assembly and method of use provides a duct sleeve with a pole adaptor hole on the side that can be attached to a new A/C/heater duct and "fished" into a narrow attic space to a register box, then left in place as the adaptor pole is unscrewed by the pole adaptor, and the pole adaptor is retrieved with a cord.
