WASHINGTON, July 17 -- Internal Revenue Service has issued a notice called: Agency Information Collection Activities; Comment Request on Forms 14039 and 14039 (SP) Identity Theft Affidavit, and Forms 14039-B and 14039-B (SP) Business Identity Theft Affidavit.

The notice was published in the Federal Register on July 17 by LaNita Van Dyke, Tax Analyst.
