Alappuzha, Feb. 20 -- Former Head of the Gynaecology Department at Alappuzha Government Medical College Hospital, Dr. Lalithambika, has described the shocking incident of a surgical instrument being found inside a woman's abdomen five years after surgery as a "system failure" rather than individual negligence.

Breaking her silence amid mounting controversy, Dr Lalithambika stated that she did not personally perform the 2021 surgery in question, though her name appeared in official records.
