Goa, Dec. 27 -- The Magisterial Inquiry Committee investigating the devastating fire at Birch by Romeo Lane nightclub has submitted its report to the State government, sources said. The inquiry attributes the tragedy, which claimed 25 lives in early December, to serious lapses in regulatory oversight and safety compliance.

Of the deceased, 20 were club staff members, while five were tourists.
