Pakistan, Oct. 16 -- A US investigation has concluded that the 2023 Titan submersible disaster, which killed five people during a dive to the Titanic wreck, was the result of poor engineering and insufficient testing. The National Transportation Safety Board (NTSB) revealed that the catastrophic implosion could have been prevented if proper design standards and safety procedures had been followed by OceanGate, the company behind the ill-fated sub.
